Hi Gurus,
Consider a forum like codeguru.com or any phpbb forums, now site admin ban a user,
but problem is he may return to the forum by creating another account, and also assume that he is on dial-up, now please give your ideas to ban this user in such a way that he is not able to get back into site by using any method like proxying,[ I hope he can use proxying on dial up ] ............etc, ;)
And one more thing please don't give idea such as finishing him off :D

If he has a dynamic IP, then you could ban the ISP's IP range. Of course this my affect other users too. Depending on the proxies you'll still get the original IP, so you could change your forum software to check for that.
But the best solution is to talk to the user to discourage him from rejoining. If all fails, you'll just have to rely on your moderators to make sure that the user doesn't come back.

They can put a cookie on the "ex-member"'s computer. Obviously these can just be cleared, but it is another message.
I really have no idea what I am talking about :P

After seeing Sahir [not visible now :p ] and your's reaction, I must clarify that this is question for just educational purpose, Actually this problem/challenge is faced by my friend who is an Admin of a forum. :) There is nothing offensive against anybody.
