grade this small db design

I had to perform a series of tests to get a job. They stated.. I have gotten the review of your tech test and the feedback was that while you presented your answers in a thorough and exemplary manner and did well on the C++ portion of the test, the Database test was not at the Lead level we need it to be at ...so I would appreciate some of you db gurus taking a look at what I submitted on the db portion. Feel free to be harsh since its important that I know. Here's my submission:
