Capture information from a list

I am creating a form using Acrobat 5. My form contains a list of objects. At the beginning of each line I used (Form tool - radio button). The user can only select one item on the list of 17. Now to the question. I would like to collect this checked data on either another form or within the same form, further down the page. So if a user were to check item 1 its definition would appear on the collection information of the form. But,
if the user were to change their mind and check another block, it would have to update the collect page info.

Any ideas? TIA
